It is necessary to submit an application orally or in writing
According to the National Tax Inspectorate in Holosiiv district in Kyiv city, blocking of enhanced certificate can be made over the telephone. An applicant, who has intent to block the electronic signature, can do it orally over the telephone calling (044) 284-00-10, however, the applicant should provide the following information to the administrator dealing with registration:
identity data of the certificate owner;
registration number of the public key certificate;
key phrase for voice authentication.
A written application for blocking the enhanced certificate is submitted to the accredited centre of key certificates or to a separate user registration point by filling out a standard form that is attested personally by the applicant. If a certificate owner is a legal person, the signature of the authorized person is certified by the company’s seal.
The forms of applications for blocking, cancelling and renewal of the enhanced certificate are available on the website of the accredited centre of key certificates, at the information and reference unit, at www.acskidd.gov.ua, sections “Forms of Documents” and “Blocking, Renewal and Cancelation of Certificates”.
